YOUR COACHING AGREEMENT

WELCOME TO YOUR PRIVATE COACHING PROGRAM AGREEMENT

This agreement is between the Coach, Carrie Eckert of Carrie Eckert, LLC dba Avocado to Zen and the Client.

This Agreement covers your program membership, constituting the following structure.

 

FREE TO LIVE 3-SESSION PRIVATE COACHING PROGRAM

This program consists of 3 x 50-minute private coaching sessions over a period of 6-8 weeks + email support in between sessions. Our program will commence following the acceptance of this agreement, the receipt of payment, and the completion of your coaching questionnaire.

The Client will call the personal Coach at the pre-arranged phone number or Zoom at the scheduled session time.

The Client will give the Coach 24 hours of notification if the session needs rescheduling. This is via text or email to carrie@avocadotozen.com. If the Client needs to re-schedule less than 24 hours in advance, or worse case, forgets the call, the session will be considered completed and will be counted towards your coaching program. If the Coach is unavailable due to unforeseen circumstances, the same notice period should be given to the Client where possible and the session will be rescheduled.

 

OUR COACHING RELATIONSHIP

Our coaching relationship is a partnership. The Coach’s sole focus is to empower the Client to attain the results he/she desires. In order to achieve the desired results, The Client must do their part by following through on commitments made.

The Client enters into the personal coaching program with the understanding that the Client is responsible for creating his/her own decisions and results. Your Coach gives specific advice and suggestions, however, it is ultimately the Client’s responsibility for making his/her own decisions and determining the best course of action.

 

Client’s Responsibilities:

  • Be on time for each session

  • Complete the questionnaire at least 24h before the second session and bring an issue, goal, or agenda to each session

  • Follow up on actions in-between meetings

  • Turn off all distractions

  • Let the Coach know if the approach is not working

 

Coach’s Responsibilities:

  • Be on time for each session

  • Hold all information completely confidential

  • Facilitate Client’s progress toward his/her goals

  • Provide tools and further resources to advance the Client’s results

 

CONFIDENTIALITY

Our personal coaching relationship is completely confidential. The Coach recognizes that the client may disclose future plans, business affairs, and personal and financial information. The Coach will not at any time share this information with a third party. The Coach recognizes that the client may have future plans, business dealings, and proprietary information. The Coach will not at any time, either directly or indirectly, use any such information for her own benefit. The Coach will not divulge that the Coach and the Client are in a coaching relationship without express consent of the client. The Client, of course, is free to discuss the coaching relationship with anyone at any time.

 

TERMINATION OF SERVICES

This is a 6-8 week commitment to your results, which the Client must take seriously. However if personal, financial, health, or other extenuating circumstances prevent the Client from completing the coaching program, the Client has the right to request to terminate this coaching agreement with 7 days written notice. Coaching will terminate when the first session has been delivered and, a refund will be issued for a maximum of $250. However, any request to terminate will still be at the Coach's discretion.

 

INDEMNIFICATION

The client shall indemnify and hold the Coach harmless from any loss or liability arising from actions taken or situations created as a result of the coaching and mentoring relationship.

 

FEE

The fee for this coaching program is $375 USD in full or 2 payments of $190.


TERM

Your personal coaching term is 6 weeks from your program commencement date plus 2 weeks to accommodate for any reschedules by the Client, or any annual leave taken by the Coach or the Client. Annual leave by the Coach will be communicated at a minimum of 2 weeks’ notice and you will be left with tools to take you through this time period.
